Clinical Clearance Form
(Yearly Physical Assessment)

Every student must update and submit their yearly Physical Assessment before starting any clinical experience including: Nursing Homes, Hospitals and Community Agencies. Completed forms are to be handed in at the Nursing Lab. Failure to submit the Physical Assessment in its entirety will result in non-clearance to participate in clinical experiences. Please download the form below and hand it in a timely manner.
